What if I cannot go online and complete the health form?

Our Reception Team will complete the same health form on your behalf during your telephone call. Once completed the medical request will be actioned in the same way as it would do if it was completed online.

Once your request has been submitted our Reception Team will discuss the online process and identify the reasons that are preventing you from using this service.

What if I have no landline number, mobile telephone or computer?

Please visit the surgery and speak to a member of our Reception Team who will complete the online form with you. At this point they will make an arrangement for you to attend to discuss the outcome.

Why can’t I log-in?

Please ensure your NHS number is correct and without spaces.

When entering your D.O.B, please ensure you are using the below criteria:

Use forward slashes ” / ” not dots ” . “

They need the full year e.g. 2025 not 25

Add zeros in front of single digit numbers

Will I get a GP appointment on the same day?

All medical requests will be assessed by a GP and appointments will be allocated according to medical need and not released on a “first come, first served basis”. 

This could mean that the GP would like to see you on the same day in a face-to-face appointment or speak to you in a telephone consultation. You might also be offered a prebooked GP appointment. Alternatively, you might be signposted to another clinician or provider who is best placed to help and support with your medical issue.

How quickly will I get a response?

The GP will action your health form and we will contact you during the same working day.

How will I be contacted?

In most cases you will be contacted via the same way that you submitted the request i.e. a text containing a booking link or containing advice/information, or via a telephone call.

You can add in your comments a preferred method of contact.

What if I am not satisfied with the response?

All medical requests and admin health forms are assessed by a GP.

Please remember that our Reception Team are only able to act upon the GP’s instruction.

Witton Street Surgery care for approximately ten thousand patients and this is why we are moving to a system which is centred around appointment allocation based upon medical need. The outcome might not always meet your desired expectation on every occasion.

Can I still get a same day appointment?

Yes, we will still be offering same day appointments. Our GPs will review each appointment request and allocate appointments based on medical needs.

How can I support the Surgery Team at this time?

Your patience and support during this time will be greatly appreciated by our Team here at Witton Street.

Please help by ensuring that all your contact details are correct and up to date. This includes your mobile telephone number, landline number and email address. It is vital that the details we hold for you are correct.

Alternatively, you can notify us in writing using the post box outside, or the internal prescriptions post box. We politely ask that you do not contact us via telephone.

Does this affect the way I request my repeat prescription?

No – please continue to submit your monthly prescription in the same way as before.

Please continue to allow 48-72 hours notice.

Can I still choose when my appointment is?

Yes you can. We will be sending out online booking links, which will show all available appointments in the timeframe the GP has requested to make an appointment. Therefore, you can choose your own appointment time.

Can I book an appointment online using the digital front door?

No, you cannot book directly into our clinics.

To book a GP appointment you need to submit a GP Appointment Request Form.

For nurses and other clinicians, we will invite you for your appointment/review when you are due. If you believe you are due for an appointment/review and we have not invited you, please call us on 01606 544850 and we will check when you are due. This includes injections and annual reviews like asthma and diabetes.

Can I book a cervical screening online?

If your cervical screening appointment with the nurse is due – please telephone our reception team, Once they have checked you are due for your screening, they will send you a booking link for you to arrange tour appointment with the nurse.

We are only able to send patients the booking link for this screening when they have received their booking invitation from NHS Cervical Screening Administration Service at the appropriate time. You will need to telephone our reception team to inform us when you receive your letter, as we do not get informed.

When is the Digital Front Door (DFD) open?

Our practice DFD follows the same opening hours as our telephone lines. Please find our telephone and DFD opening hours below:

Monday: 08:00 am to 06:30 pm

Tuesday: 08:00 am to 06:30 pm

Wednesday: 08:00 am to 06:30 pm

Thursday: 08:00 am to 06:30 pm

Friday: 08:00 am to 06:30 pm

Saturday: Closed

Sunday: Closed

How do I book my routine reviews and non-GP appointments?

Our practice will invite you for your routine reviews and non-GP appointments. We will invite you by SMS/text, email, letter, or phone call.

The reviews and appointments we will invite you for include but are not limited to: Asthma, CHD, Childhood Immunisations, COPD, Diabetes, Heart Failure, Hypertension, NHS Health Checks, Postnatal Discharge Reviews, etc

Do I need to download an app or create an account?

No, you do not need to download any apps or create an account for our new system. It is an online webpage, you will need your date of birth and NHS number to access it.
